About Quick Reports

Even though the text area is small, you can enter up to 200 characters and the text will scroll to the left. Why didn't I put in a multi-line text area? It is because there are these spam robots that search around for forms with text areas. On a simple form like this, it would be hard to stop them without using a captcha, which is that thing that shows an image of squiggly letters that you must type in before you can submit the form.

For general information questions, call the Registrar of Contractors, Information Center. When you reach the agency through either of the numbers above, you can speak to a Customer Service Representative during business hours (Monday through Friday, 8 a.m. to 5 p.m., MST). You may obtain information on licensed contractors, request complaint forms or license application packets by contacting an Information Center Customer Service Representative. Satellite offices may experience intermittent closing for a variety of reasons. In the event a satellite office is closed and you need immediate assistance please contact any of our regional offices or the main office in Phoenix.

Customer Service Representatives in the Information Center provide general information on licensed contractors, basic licensing and testing requirements, basic administrative procedures on complaints and licensing, and will direct calls to other agency personnel as needed.

The information provided by the Customer Service Representatives about licensed contractors includes the company name, license number, classification title, issue dates, status of the license, complaint status (if any), previous licenses and issue date, the names of the owners, partners or corporate officers, the qualifying party, and the company address.

Bonding information is also available, including the name of the bonding company, bond number, bond amount, effective date, cancellation date if applicable.

Arrangements for viewing complaint files at either the Phoenix, Glendale, Tucson or Mesa offices can be initiated by contacting a Customer Service Representative at the Information Center.

If you are calling for information about a licensed contractor it is helpful to have the license number ready. This will help the Customer Service Representative look up the information you need. The exact name of the contractor, including the spelling and punctuation used will expedite retrieval of the information
