Any change to information provided in order to obtain a contractors license must be provided to the Registrar of Contractors within 30 calendar days of the change.

Use this form to change the company mailing address, this must be done within 30 calendar days of the move. Mail original form with original signature to P.O. Box 6688, Phoenix,AZ 85005-6688.

[info] Application to Waive a Trade Exam RC-L-310 (2/15)F&S  
Use this form to request a waiver of the trade exam.

[info] Assumption RC-L-242 (7/12) F&S
Use this form to assume responsibility of any license being canceled when applying for a new license. One (1) copy is required for each license being applied for, to be sent to any office of the Arizona Registrar of Contractors.

You may furnish this form to your insurance company, licensed to write bonds in the State of Arizona. See bond instructions below.

[info] Cancellation Request RC-L-241 (7/03)
Use this form to cancel (terminate) a license. One (1) copy may be sent to any office of the Arizona Registrar of Contractors, that single copy may list all of the licenses you wish to cancel. A canceled license can not be reactivated.

Use these forms/instructions when a Criminal History disclosure is required. One (1) copy is required to be sent to any office of the Arizona Registrar of Contractors.

[info] Exemption from Qualifying Party Requirements Application RC-L-280 (5/13)F&S  
Use this form to apply for a qualifying party exemption. Each license requires a separate form and fees. Be sure you meet all of these requirements before applying for an exemption.

[info] Experience-Project/Lics RC-L-200-S1 (2/15)F&S  
Use this form to record experience of a qualifying person. One (1) copy is required for each employer worked for and include the experience documentation in your license application.

[info] Financial Statement RC-L-201 (12/14)
Use this form to report the financial standing of a company when applying for a new commercial or swimming pool contractors license. One (1) copy is required to be included in each license application.

[info] Inactive Form RC-L-214 (2/15)F&S  
Use this form to place a license inactive. One (1) completed form is required for each license to be placed in inactive status. Inactivating a license keeps it from expiring during a period when it is not intended to be used. It can then be reinstated within five (5) years of the inactivation date.

Use this form if you wish to change the company name listed on your license, one (1) copy is required to be sent to any office of the Arizona Registrar of Contractors.

[info] Name Change Request for Certificates of Deposit RC-L-293 (7/12)
If a Certificate of Deposit is held in lieu of the surety bond, include this form instead of a bond rider. Please include one completed form for each license number.

New Qualifying Party Application Line-by-Line QP Instructions [info] QP Application Forms RC-L-205 (3/15) F&S
Use this form for replacing the qualified party of any license. One (1) copy is required for each license the qualifying party is being changed on, to be sent to any office of the Arizona Registrar of Contractors.

[info] Notice of Disassociation/Resignation RC-L-305 (2/15) F&S 
Use this form to report the resignation of a qualifying party. Any officer, partner or member of a limited liability company. One (1) copy is required to be sent to the [Phoenix office] of the Arizona Registrar of Contractors.

[info] Personnel Change Form; RC-L-286 (12/14) F&S
Use this form to report any change of personnel or change in the percentage of ownership. One (1) copy is required to be sent to any office of the Arizona Registrar of Contractors. (This form can be filled in and saved)

[info] Reinstatement Packet RC-L-222 (2/15)F&S
Use this form to reinstate a license that has been placed on inactive status. One (1) copy is required for each license to be reinstated, and the Arizona Registrar of Contractors must have 30 days notice before the license is due to expire.

[info] Request for Affidavit RC-L-303 (2/15)
Use this form to request an affidavit with regards to information concerning any license. One (1) copy is required, to be sent to any office of the Arizona Registrar of Contractors with a $10.00 processing fee.

Use this form to show proof of compliance with the workers compensation act. One (1) copy is required to obtain or renew a contractors license.
Name Request, License Application & Candidate Information Bulletin
[info] Name Request and Availability F&S
Use this form to reserve a company name prior to applying for a license. One (1) copy is required to be sent to any office of the Arizona Registrar of Contractors. The name, if available, will be reserved for 90 calendar days. It is also advisable to verify that the name is available with the Secretary of State and the Corporation Commission.
