Use this form to apply for a qualifying party exemption. Each license requires a separate form and fees. Be sure you meet all of these requirements before applying for an exemption.

Use this form to place a license inactive. One (1) completed form is required for each license to be placed in inactive status. Inactivating a license keeps it from expiring during a period when it is not intended to be used. It can then be reinstated within five (5) years of the inactivation date.

If a Certificate of Deposit is held in lieu of the surety bond, include this form instead of a bond rider. Please include one completed form for each license number.

Use this form to reserve a company name prior to applying for a license. One (1) copy is required to be sent to any office of the Arizona Registrar of Contractors. The name, if available, will be reserved for 90 calendar days. It is also advisable to verify that the name is available with the
Secretary of State
and the
Corporation Commission.

Use this form to verify a Nevada license is held by a qualifying individual for the purpose of waiving the experience and trade test requirements. One (1) original is required, must be included in your license application.

Use this form to report any change of officers or change in the percentage of ownership of a corporation. One (1) copy is required to be sent to any office of the Arizona Registrar of Contractors.
[info] Reinstatement RC-L-222 (1/09)
Use this form to reinstate a license that has been placed on inactive status. One (1) copy is required for each license to be reinstated, and the Arizona Registrar of Contractors must have 30 days notice before the license is due to expire.

Use this form to request an affidavit with regards to information concerning any license. One (1) copy is required, to be sent to any office of the Arizona Registrar of Contractors with a $10.00 processing fee.
[info] Request for Certified Copies RC-L-304 (1/09)
Use this form to request certified copies of any license or license information. Certified copies are required for many reasons and when a certified copy is issued it indicates that the Registrar of Contractors has made a copy of the document and that it is a true and accurate copy of the information on that document. A $10.00 processing fee is required.
