Any change to information provided in order to obtain a contractors license must be provided to the Registrar of Contractors within 30 calendar days of the change.

Bond Form RC-L-220D (2/08)

Cancellation Request RC-L-241 (7/03)

close X Use this form to cancel (terminate) a license. One (1) copy may be sent to any office of the Arizona Registrar of Contractors, that single copy may list all of the licenses you wish to cancel. A canceled license can not be reactivated.

Financial Statement RC-L-201 (7/03)

Address Change RC-L-302 (02/08)

close X Use this form to change the company mailing address. One (1) original is required, to be sent to any office of the Arizona Registrar of Contractors within 30 calendar days of the change.

Assumption RC-L-242 (7/03)

close X Use this form to assume responsibility of any license being canceled when applying for a new license. One (1) copy is required for each license being applied for, to be sent to any office of the Arizona Registrar of Contractors.

California Reciprocity RC-L-311 CA (2/08)

close X Use this form to verify a California license is held by a qualifying individual for the purpose of waiving the experience and trade test requirements. One (1) original is required, must be included in your license application.

Exemption from Qualifying Party Requirements Application RC-L-280 (02/08)

close X Use this form to apply for a qualifying party exemption. Each license requires a separate form and fees. Be sure you meet all of these requirements before applying for an exemption.

Experience/Lics RC-L-200-S1 (2/08)

close X Use this form to record experience of a qualifying person. One (1) copy is required for each employer worked for and include the experience documentation in your license application.

Felony/Lics Package RC-C-061I and RC-C-61A (02/08)

Inactive Form RC-L-214 (2/08)

close X Use this form to place a license inactive. One (1) completed form is required for each license to be placed in inactive status. Inactivating a license keeps it from expiring during a period when it is not intended to be used. It can then be reinstated within five (5) years of the inactivation date.

Name Change Request RC-L-300 (2/08)

close X Use this form if you wish to change the company name listed on your license, one (1) copy is required to be sent to any office of the Arizona Registrar of Contractors.

Name Reservation RC-L-301 (2/08)

close X Use this form to reserve a company name prior to applying for a license. One (1) copy is required to be sent to any office of the Arizona Registrar of Contractors. The name, if available, will be reserved for 90 calendar days. It is also advisable to verify that the name is available with the Secretary of State and the Corporation Commission.

Nevada Reciprocity RC-L-311 NV (2/08)

close X Use this form to verify a Nevada license is held by a qualifying individual for the purpose of waiving the experience and trade test requirements. One (1) original is required, must be included in your license application.

New Qualifying Party Application RC-L-205 (2/08)

close X Use this form for replacing the qualified party of any license. One (1) copy is required for each license the qualifying party is being changed on, to be sent to any office of the Arizona Registrar of Contractors.

Notice of Disassociation RC-L-305 (2/08)

close X Use this form to report the resignation of a qualifying party. Any officer, partner, or member of a limited liability company. One (1) copy is required to be sent to the [Phoenix office] of the Arizona Registrar of Contractors.

Officer Change Form; RC-L-286 (2/08)

close X Use this form to report any change of officers or change in the percentage of ownership of a corporation. One (1) copy is required to be sent to any office of the Arizona Registrar of Contractors.

Reinstatement RC-L-222 (02/08)

close X Use this form to reinstate a license that has been placed on inactive status. One (1) copy is required for each license to be reinstated, and the Arizona Registrar of Contractors must have 30 days notice before the license is due to expire.

Request for Affidavit RC-L-303 (2/08)

close X Use this form to request an affidavit with regards to information concerning any license. One (1) copy is required, to be sent to any office of the Arizona Registrar of Contractors with a $10.00 processing fee.

Request for Certified Copies RC-L-304 (2/08)

close X Use this form to request certified copies of any license or license information. Certified copies are required for many reasons and when a certified copy is issued it indicates that the Registrar of Contractors has made a copy of the document and that it is a true and accurate copy of the information on that document. A $10.00 processing fee is required.

Social Security Form RC-L-223 (2/08)

close X Sole proprietors may use this form to complete a license application. One (1) copy is required to accompany your license application to be sent to any office of the Arizona Registrar of Contractors. For license renewals. One (1) copy RC-L-314R (3/00) is required to accompany your license renewal.

Utah Reciprocity RC-L-311 UT (2/08)

close X Use this form to verify a Utah license is held by a qualifying individual for the purpose of waiving the experience and trade test requirements. One (1) original is required, must be included in your license application.

Workers Comp RC-L-317A (2/08)

close X Use this form to show proof of compliance with the workers compensation act. One (1) copy is required to obtain or renew a contractors license.
